The COMLEX, or, Comprehensive Osteopathic Medical Licensing Examination, is a set of three standardized medical exams known as Level 1, Level 2, and Level 3. COMLEX Level 1, the first of the COMLEX exams, is intended to assess student readiness for lifelong practice-based learning as a Doctor of Osteopathic Medicine.


Level 1 is considered to be one of the most important and most challenging tests in an osteopathic medical student’s career and is often taken within days or weeks of USMLE Step 1. COMLEX Level 1 is scored as pass/fail. Score reports have two sections: The status of “Pass” or “Fail,” and the Performance Profile that provides performance information to the examinee (and the school) for the purposes of continuous quality improvement and lifelong learning.

COMLEX Level 1 is a one-day, computer-based exam with 352 questions split into two four-hour sessions, plus a 40-minute break. Each session includes an additional 10-minute break. Effective time management and familiarity with the format are essential for success.


A structured six-month study plan makes preparation more manageable. The first two months focus on foundational review of high-yield subjects like anatomy, physiology, and osteopathic principles. Months three and four emphasize question-based learning, identifying weak areas, and taking full-length practice exams. The final phase prioritizes test-taking strategies and additional timed practice exams.
