The COMLEX, or, Comprehensive Osteopathic Medical Licensing Examination, is a set of three standardized medical exams known as Level 1, Level 2, and Level 3. Level 3, the final exam in the COMLEX series, is designed to assess student competencies required for generalist physicians to deliver safe and effective osteopathic medical care in unsupervised clinical settings. According to the NBOME website, “ [Level 3] competency assessment occurs in the context of clinical and patient presentations and systems-based practice as required for entry into the unsupervised practice of osteopathic medicine as an independently practicing osteopathic generalist physician and for readiness for lifelong learning and practice-based learning and improvement.”

What is COMLEX Level 3?

Even though COMLEX Level 3 is the exam that ultimately allows doctors of osteopathic medicine to practice in the United States, it is generally regarded as the least important of the COMLEX exams. This is because COMLEX Level 3 is taken after internship/residency has already begun, thus making it much less stressful than the others. However, many students do struggle with the length of Level 3, as it is nearly twice as long as Levels 1 and 2-CE.

The examination also includes audio-visual components designed to assess an expanded physician competency subset.
